What is SMILE?

SMILE (Small Incision Lenticule Extraction) is a minimally invasive laser vision correction procedure that effectively treats nearsightedness and astigmatism. It is the only form of LALEX (Laser-Assisted Lenticule Extraction) available in the United States, and is the newest form of laser vision correction available.

MVC is the only location in the Des Moines area offering the SMILE procedure. At MVC, Dr. Dickson performs SMILE using the ZEISS VisuMax femtosecond laser, which creates a thin, disc-shaped piece of corneal tissue, known as a lenticule, beneath the surface of the eye. Removing this lenticule reshapes the cornea to correct vision. Unlike LASIK, SMILE does not involve creating a flap or removing corneal tissue through ablation, making it a streamlined, single-step procedure. This advanced technique offers precision, safety, and stability while providing an excellent option for patients to fix their vision. As one of the most advanced forms of laser vision correction available today, SMILE is exclusively performed at select premier eye surgery centers across the country, and MVC is proud to offer this innovative solution to patients seeking lasting visual freedom.

ZEISS VisuMax Technology

The ZEISS VisuMax femtosecond laser is a state-of-the-art system that sets a new standard in refractive surgery. Known for its precision, speed, and gentle approach, this advanced laser technology is designed to perform highly accurate corneal procedures with minimal disruption to surrounding tissue. Unlike traditional laser systems, the VisuMax creates exceptionally smooth and precise incisions, enhancing both safety and patient comfort. Its engineering allows for a more delicate treatment experience, reducing pressure sensations commonly associated with other laser technologies.

As the first and only laser in the United States capable of performing SMILE surgery, the VisuMax represents a breakthrough in vision correction. Its ability to sculpt and remove a tiny lenticule of corneal tissue provides a seamless and efficient approach to reshaping the cornea. This technology is widely used in leading eye centers around the world and has been FDA-approved in the United States since 2016. At MVC, this cutting-edge laser technology allows our surgeon to offer patients a more precise, comfortable, and effective vision correction experience.

How does SMILE eye surgery work?

SMILE eye surgery works by using a femtosecond laser to reshape the cornea and correct refractive errors like nearsightedness and astigmatism. The procedure begins with the ZEISS VisuMax femtosecond laser creating a thin, lens-shaped layer of tissue, called a lenticule, just beneath the surface of the cornea. This precise laser process takes only about 10 seconds per eye and does not disturb the outer corneal surface. Once the lenticule is formed, the laser also creates a small incision, typically 4 millimeters or less, through which the surgeon carefully removes the lenticule. Extracting this tiny piece of tissue alters the shape of the cornea, improving how light is focused onto the retina and resulting in clearer vision. Because SMILE does not require a corneal flap, the procedure is minimally invasive, reducing the risk of dry eye. The small incision heals naturally within a few days without the need for stitches, allowing patients to return to normal activities quickly. MVC utilizes this advanced laser technology to provide a precise, effective, and comfortable vision correction experience for our patients.

SMILE Eye Surgery: What to Expect

SMILE eye surgery is a quick, outpatient procedure that takes only a few minutes per eye. Before the surgery, anesthetic eye drops will be applied to numb the eyes, and a mild sedative may be offered to help you relax. A small device will gently keep your eyes open, so there’s no need to worry about blinking. After the procedure, you’ll need a friend or family member to drive you home, as your vision may be blurry for a few hours. Most patients notice improvements in their vision the same day, with stabilization over the next few days.

Recovery after SMILE is typically smooth and fast. Since there is no corneal flap, patients experience less discomfort and a shorter period of temporary dryness compared to LASIK. While vision may fluctuate initially, many people can return to work, drive, and resume daily activities as soon as the next day. Prescription eye drops will be provided to reduce inflammation and prevent infection, and it’s important to follow all post-operative instructions to ensure proper healing. Follow-up appointments will be scheduled to monitor progress and confirm that vision is improving as expected. Within one week, most patients can wear makeup, exercise, and even swim. SMILE eye surgery at MVC offers a convenient, effective way to achieve long-term visual freedom with minimal downtime.

What is the difference between SMILE and LASIK Surgery?

SMILE and LASIK are both laser vision correction procedures, but they use different techniques to reshape the cornea. The biggest difference is that LASIK involves creating a corneal flap, while SMILE is a flapless procedure. During LASIK, a femtosecond laser creates a thin flap in the cornea, which is lifted so an excimer laser can reshape the underlying tissue. The flap is then placed back in position, where it heals naturally. SMILE, on the other hand, uses a femtosecond laser to create a small, lens-shaped piece of tissue, called a lenticule, within the cornea. This lenticule is removed through a tiny incision, allowing the cornea to reshape without the need for a flap.

Because SMILE does not require a corneal flap, it results in a smaller incision—up to 80% smaller than the flap created in LASIK. This means the cornea remains more biomechanically stable, and there is a shorter period of temporary dry eye symptoms after surgery. SMILE is also a single-laser procedure, making it streamlined and efficient. While LASIK can treat a wider range of prescriptions, including farsightedness, SMILE is specifically FDA-approved for nearsightedness and astigmatism. Both procedures offer excellent vision correction, and the best option depends on individual eye health, prescription, and lifestyle. The MVC team conducts a thorough evaluation on each patient to determine whether SMILE, LASIK, or another vision correction surgery is the right choice for achieving clear, long-lasting vision.

FAQ's About SMILE

How much does SMILE eye surgery cost?
The cost of SMILE amongst different practices varies and depends on several factors, including the extent of the screening examination, the technology used, the expertise of your surgeon, and the patient experience. While pricing can vary, MVC offers rates that reflect the high level of care, advanced technology, and precision that goes into every procedure. Be wary of the discount providers that may cut corners on technology or experience. At MVC, we prioritize patient safety, experience, and long-term results. Like most things in life, the quality of your SMILE experience is reflective of its price. Many patients find that SMILE is a smart financial investment, eliminating the ongoing costs of glasses, contacts, and maintenance over the years. SMILE is like buying your vision, while contacts and glasses are like renting your vision. During your consultation, we will provide a personalized cost estimate and discuss financing options to make clear vision as accessible as possible.

Most insurance plans do not cover SMILE because it is considered an elective vision correction procedure rather than a medically necessary treatment. However, some employer-sponsored health savings accounts (HSAs) or flexible spending accounts (FSAs) can be used to help with the cost. MVC provides guidance on financing and payment options to ensure patients can access this advanced, minimally invasive procedure. Checking with your insurance provider and exploring available financial resources can help make SMILE a more affordable option.

Yes, SMILE is an effective solution for treating both myopia and astigmatism. The precision of the Zeiss VisuMax femtosecond laser allows for customized reshaping of the cornea, improving visual perception and reducing dependence on glasses or contact lenses. Unlike some older procedures, SMILE’s advanced laser technology ensures accuracy in treating refractive errors while preserving more of the eye’s natural structure. Patients with up to 3.00 dioptres of astigmatism can achieve clear, stable vision with this procedure at MVC.
As with any surgical procedure, SMILE has potential risks, though they are minimal. Some patients may experience temporary symptoms such as light sensitivity, glare, or halos during the healing process. The duration of temporary of dry eye symptoms is lower compared to LASIK because SMILE involves a smaller surgical incision, which helps preserve corneal nerves responsible for tear production. At MVC, detailed pre-operative evaluations, including corneal topography, ensure patients receive a personalized treatment plan to minimize risks and optimize results.
Most patients notice significant visual improvement within hours of surgery, and vision continues to sharpen over the next couple days. While minor fluctuations are normal, many return to daily activities, including work and driving, within 24 hours. Unlike photorefractive keratectomy (PRK), which has a longer healing time, SMILE offers a much faster recovery due to its flap-free design. At MVC, post-operative care is tailored to support a smooth and efficient recovery process, ensuring optimal visual results.
SMILE is a comfortable experience, with anesthetic eye drops used to numb the human eye before the procedure begins. Most patients report feeling slight pressure but no pain. The VisuMax laser uses advanced mode-locking technology to ensure precise and rapid treatment, minimizing discomfort. After surgery, some patients may experience mild irritation or sensitivity, which typically resolves within a few hours. MVC prioritizes patient comfort and provides thorough guidance on post-operative care to ease the recovery process.
Yes, SMILE is typically performed on both eyes during the same appointment for a convenient and efficient treatment experience. This approach allows for a balanced recovery, as both eyes adjust simultaneously to their new vision. Since SMILE is a minimally invasive procedure, patients can resume most normal activities quickly. At MVC, the surgical team ensures each patient receives personalized care to achieve the best possible outcome with minimal downtime.
SMILE and PRK are both effective options for vision correction, but they use different techniques. PRK, or photorefractive keratectomy, removes the corneal epithelium to reshape the underlying tissue with an excimer laser, requiring a longer healing period. SMILE, by contrast, creates a small incision and removes a lenticule from within the cornea without affecting the surface, leading to faster recovery and less discomfort. Patients at MVC benefit from a thorough consultation to determine which procedure best suits their visual needs and lifestyle.
Although all vision correction procedures may cause some temporary dry eye symptoms, this is less prevalent with SMILE compared to LASIK because it does not require creating a corneal flap. This means fewer corneal nerves are disrupted, preserving the eye’s natural tear production and reducing the risk of dry eye symptoms. Patients who have experienced signs and symptoms of dry eye before surgery often find SMILE to be a preferable option. At MVC, advanced diagnostics help identify patients who may be at higher risk for dryness and determine the best course of treatment.
Some patients who are not eligible for LASIK may still qualify for SMILE. Because SMILE does not involve creating a flap, it maintains more of the corneal structure, making it a safer option for certain individuals. The Food and Drug Administration has approved SMILE for treating myopia and astigmatism, providing more patients with an effective alternative to traditional laser vision correction. MVC offers comprehensive evaluations to determine the most suitable vision correction procedure for each patient’s unique needs.
